Cloquet, Minnesota
Sand Creek Workplace Wellness

The Employee Assistance Program (EAP) recognizes that from time to time we all experience serious personal problems. These may include marital, financial, drug or alcohol, vocational, and stress-related difficulties. Sometimes these problems affect our job performance, sometimes just our general well being.  The important thing is that you don't have to solve these problems alone.

The City offers its employees, household family members, and dependent children living outside the home help through the Employee Assistance Program.  The EAP is a benefit provided by the City for you and your family-just for that reason-you don't have to go it alone.

The EAP is confidential and your right to privacy is strictly protected. All visits with the Employee Assistance staff are available to you, household family members, and dependent children living outside the home.  Initial visits and counseling sessions are paid by the City as detailed in the City's plan. Currently the Employee Assistance Program is contracted to Sand Creek Workplace Wellness.
Help is as simple as a phone call. You or a family member can call and speak with one of the counselors, who will schedule an appointment.  There is a counseling office in Duluth at the Northland Medical Center South, 925 East Superior Street.  They can be reached 24 hours a day at (218)481-7477.
The City of Cloquet considers its employees a valuable resource and knows how important your family is to you. When you or a family member are experiencing problems, please give Sand Creek a call. Your well being and that of your family are their first consideration.

For those who wish to use the internet, you can request an appointment through there website at  In addition, we urge you to explore the special wellness website within Sandcreek's website.  Check with your supervisor or human resources for the company ID that will allow you to access this helpful site.
